37THIS CAUSE came on for consideration before the Agency for Health Care Administration

50("the Agency"), which finds and concludes as follows:

601. The Agency issued the Petitioner a letter denying his application for exemption from

74disqualification from employment. (Exhibit A).

792. The Petitioner requested a formal administrative hearing, and his petition was sent to the

94Division of Administrative Hearings (DOAH).

993. The Petitioner subsequently filed a Notice of Voluntary Dismissal with DOAH (Exhibit

112B), and DOAH closed its file and relinquished jurisdiction to the Agency (Exhibit C).

1264. The denial of the Petitioner's application for exemption from disqualification from

138employment is UPHELD.

1415. This Final Order is entered without prejudice to Petitioner reapplying for an exemption

155from disqualification from employment in the future.
