FINAL ORDER

THIS CAUSE carne on for consideration before the Agency for Health Care Administration,

which finds and concludes as follows:

1. The Agency issued the Respondent the attached administrative compliant (Ex. 1 ). The

parties have entered into the attached Settlement Agreement (Ex. 2). The Settlement Agreement is

approved and adopted as part of this Final Order.

2. The Respondent shall pay the Agency ($55,000). The initial payment of fifteen thousand

dollars ($15,000) was due and received by certified funds at the time of execution of the settlement

agreement. The remaining fine of forty thousand dollars ( 40,000) shall be due and payable in eight equal

monthly payments of five thousand dollars ($5,000.00) each from the Respondent to the Agency. The

first of these monthly payments shall be due on February 1, 2015. The remaining seven payments shall

be due on the last day of each of the seven successive months. Unpaid amounts are subject to statutory

interest and may be referred to collections.

2. Checks should be made payable to the "Agency for Health Care Administration." The

checks and a reference to the ten-digit AHCA number should be sent to:

3. Any requests for an administrative hearing are withdrawn. The parties shall bear their

own costs and attorney's fees. This matter is closed.

NOTICE OF RIGHT TO JUDICIAL REVIEW

A party that is adversely affected by this Final Order is entitled to seek judicial review which shall be

instituted by filing one copy of a notice of appeal with the agency clerk of AHCA, and a second copy,

along with filing fee as prescribed by law, with the District Court of Appeal in the appellate district

where the agency maintains its headquarters or where a party resides. Review of proceedings shall be

conducted in accordance with the Florida appellate rules. The notice of appeal must be filed within 30

days of rendition of the order to be reviewed.
