53THIS CAUSE came on for consideration before the Agency for Health Care Administration

751. The Agency issued the Petitioner the attached Notice of Intent to Deny Renewal

89Application ( Ex. 1). The parties have since entered into the attached Settlement Agreement ( Ex. 2),

1242. The Petitioner' s initial application for licensure is withdrawn.

1343. The Petitioner shall pay the Agency an administrative fine of $ 2, 00. 500. If full payment

170full payment has not been made, payment is due within 30 days of the entry of the Final Order. A check
