69THIS CAUSE came on for consideration before the Agency for Health Care Administration (" the

84Agency"), which finds and concludes as follows:

921. The Agency issued the Petitioner the attached Second Amended Notice of Intent to Deny

107Renewal Application. ( Ex. 1). The parties have since entered into the attached Settlement Agreement

122Ex. 2), which is adopted and incorporated by reference.

1312. The parties shall comply with the terms of the Settlement Agreement.

1433. The Petitioner shall pay the Agency $ 1, 500. 00. If full payment has been made, the

161cancelled check acts as receipt of payment and no further payment is required. If full payment has not

179been made, payment is due within 60 days of the Final Order. Overdue amounts are subject to statutory

197interest and may be referred to collections. A check made payable to the " Agency for Health Care

283A party that is adversely affected by this Final Order is entitled to seek judicial review which shall be
