FINAL ORDER APPROVING SETTLEMENT AGREEMENT

This cause is before the Agency for Persons with Disabilities (“Agency”) for

entry of a Final Order Approving Settlement Agreement entered into between the

APD – Angel Heart Support Services, Inc. FO Page 2 of 5

Agency and Angel Heart Support Services, Inc. (“Respondent”), which is attached

as Exhibit A.

On January 23, 2020, the Agency filed Administrative Complaints against

each of Respondent’s four Agency - licensed group homes. The license numbers for

those group homes are 11 - 1106 - GH, 11 - - 1142 GH, 11 - 1178 - GH, and 11 - 1258 - GH.

On February 20 , 2020 , Responden t timely filed a Request for Administrative

Hearing for license number 11 - 1258 - GH and the case was referred to the Division

of Administrative Hearings (“DOAH”) . After an evidentiary hearing and vacating

Final Orders pertaining to the other three group homes, all four cases were referred

to DOAH. On September 3 0 , 2020 , the Agency filed an Unopposed Motion to

Relinquish Jurisdiction based on the parties’ resolution of the case s without the need

for a hearing , which the administrative law judge granted on October 1, 2020.

Upon consideration of the attached Settlement Agreement approved by the

Agency on October 2, 2020 , and being fully advised in the premises, it is he reby

ORDERED AND ADJUDGED:

1. The Settlement Agreement is hereby adopted and incorporated by

reference.

2. The parties shall adhere to and abide by all the terms and conditions of the

Settlement Agreement .

3. This Final Order shall take effect upon filing ith the Clerk of the Agency

for Persons with Disabilities.

4. Any violation of this Settlement Agreement is considered a violation of

the Final Order.
