35Having reviewed the Administrative Complaint ("Complaint"), the Amended Notice of

721. The Agency issued the attached Complaint, Notice, and Election of Rights forms

85to Love Seniors Home, Inc. d/b/a Paradise Island and Associates ALF ("Provider"). (Comp. Ex.

1011 ). The parties have since entered into the attached Settlement Agreement (Ex. 2), which is adopted

118and incorporated by reference into this Final Order.

126Based upon the foregoing it is ORDERED:

1332. The findings of fact and conclusions of law set forth in the Administrative

147Complaint are adopted and incorporated by reference into this Final Order.

1583. Licensure for Love Seniors Home, Inc. d/b/a Paradise Island and Associates ALF,

171license number 12365, is surrendered.
