19A-1. Local Municipal Bond Reporting Procedures |
1(1) Each Unit of Local Government shall furnish the Division with a report containing a complete description of each outstanding issue of General Obligation or Revenue Bond28s. The report shall follow the format and provide the following information as prescribed in Form BF 2003, “Bond Information Form,” effective 3-3-83, available from the Division, 312 Knight Building, 2737 Centerview Drive, Tallahassee, Florida 32399-0950:
65(a) The name, type, mailing address and county of the Unit of Local Government;
79(b) The name of the bond issue;
86(c) The amount of bonds authorized;
92(d) The amount of bonds issued;
98(e) The dated date, sale date and delivery date of the bond issue;
111(f) The legal authority for issuance;
117(g) The type and purpose of the issue;
125(h) The name, dated date and original par value of prior bond issues refunded by the bonds about which information is being provided;
148(i) The specific revenues pledged for payment of debt service on the bonds;
161(j) The type of sale;
166(k) The basis of interest rate calculation and rate;
175(l) Insurance;
177(m) Bond ratings;
180(n) The financial advisor to the Unit of Local Government in connection with the bond issue;
196(o) Bond counsel;
199(p) The Managing Underwriter for the bond issue;
207(q) Paying agent;
210(r) Registrar;
212(s) Maturity schedule;
215(t) Optional redemption provisions;
219(u) The name, title and telephone number of the person completing the Bond Information Form; and
235(v) The date the Bond Information Form is completed.
244(2) Although the Unit of Local Government is under a continuing duty to provide the report contemplated by this subsection, the Division may, from time to time, request such report from the Unit of Local Government. In the event such a request is made the report shall be forwarded to the Division within one hundred twenty (120) days after any such request.
