28-106.402. Contents of Request for Mediation

Effective on Tuesday, February 5, 2013

1The request for mediation shall contain:

7(1) The name, address, any e-mail address, and telephone number of the party requesting mediation and of that party’s representative, if any; and

30(2) A statement of the preliminary agency action.
