1(1) The following forms and instructions are incorporated by reference and are used by the Commission in its dealings with the public:
23(a) Form 2, Quarterly Client Disclosure. These are the fields of information required to be utilized by elected constitutional officers, state officers, local officers, and specified employees for compliance with Section 54112.3145(6), F.S., 56as set forth by instructions available at 63http://www.flrules.org/Gateway/reference.asp?No=Ref-1582165. 68(b) Form 50, Complaint. 149(c) Form 3A, Statement of Interest in Competitive Bid for Public Business. To be utilized by persons wishing to do business with their agency or to have employment or a contractual relationship with a business entity doing business with their agency, provided that the business is awarded under a system of sealed, competitive bidding, under Section 205112.313(12)(b), F.S. 207Effective 1/2007.
209(d) Form 4A, Disclosure of Business Transaction, Relationship, or Interest. To be utilized by advisory board members who wish to have certain conflicts of interest waived by their appointing authority as provided in Section 243112.313(12), F.S.; 245to be utilized by persons wishing to do business with their agency, or to have employment or a contractual relationship with a business entity doing business with their agency, where the business entity involved is the only source of supply within the political subdivision, under Section 291112.313(12)(e), F.S.
