53ER13. Emergency Rule For Year 2013 |

I1204f the Florida Lottery is unable to contact a first prize winner within three weeks of the date of the drawing, the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to an alternate winner. 1249This process will continue until an alternate is contacted or the Florida Lottery has exhausted the list of available alternates. If the Lottery is unable to contact an alternate, the first prize will not be awarded. An alternate winner will be awarded the difference in cash between the first and second prizes.
13012. Second Prize. 1304If the Florida Lottery is unable to contact a second prize winner within three weeks of the date of the drawing, the winner will forfeit his or her right to claim the prize. 1337A second prize winner who forfeits his or her right to claim a second prize will also forfeit his or her right to serve as an alternate for a first prize.

1792(a) 1793To claim 1795a 1796grand, first or second 1800prize in a 1803LOTTO LOVE 1805drawing, 1806the 1807winner 1808must submit 1810to 1811the Florida Lottery the original valid 1817entry 1818voucher bearing the unique number selected in the drawing1827. 1828Without such voucher, the 1832winner 1833will forfeit his 1836or her right to claim a prize. 1843Winners must submit the valid entry voucher along with 1852a completed Winner Claim Form DOL-173-2, revised 02/11, or DOL-173-2S, revised 02/11, 1864and 1865a copy of acceptable identification1870. 1871Forms DOL-173-2 and 1874DOL-173-2S 1875are hereby incorporated by reference and can be obtained 1884from any Lottery office, 1888from the Lottery’s website, 1892www.flalottery.com, 1893or by writing to: Florida Lottery, Customer Service, 250 Marriott Drive, Tallahassee, Florida 32399-4016. 1907The claim form must be received by the Florida Lottery no later than one week after the winner is notified by the Florida Lottery that he or she is a winner. If the Florida Lottery has not received the claim form by the seventh day after notification, the winner will forfeit his or her right to claim the prize1966. 1967If the 1969forfeited 1970prize is a grand prize or first prize, the 1979Florida Lottery will award the prize to an alternate winner as described in subsections (199451995) and (199861999) above.
2001(b) 2002To claim a 2005third 2006prize 2007in a LOTTO LOVE drawing, 2012the 2013winner 2014must 2015submit 2016to the Florida Lottery 2020the 2021original 2022valid entry voucher bearing the unique number selected in the drawing2033. 2034Without such voucher, the winner will forfeit his or her right to claim a prize. Winners must submit the valid entry voucher along with a copy of acceptable identification. 2063The documents must be received by the Florida Lottery no later than one week after the winner is notified by the Florida Lottery that he or she is a winner. If the Florida Lottery has not received the 2101documents 2102by the seventh day after notification, the winner will forfeit his or her rig2116ht to claim the prize.

2188(c) Second Prize. Upon receipt of a second prize winner’s required documentation, the Florida Lottery will award a prize of $2,500.

2231(223292233) Except as specifically mentioned herein, all federal, state and/or local taxes or other fees 2248on LOTTO 2250LOVE 2251Promotion 2252prizes will be the responsibility of the winner. 2260Federal income taxes are required to be withheld from a prize awarded to a nonresident alien claimant at the rate of thirty percent (30%) pursuant to applicable provisions of the Internal Revenue C2293ode.
2294(2295102296) If the winner of a 2302grand, first or second 2306prize is identified as owing an outstanding debt to a state agency or child support collected through a court, the debt will be collected in accordance with 2333S2334ection 233524.115, 2336F.S2337.
