585(a) 586If the Florida Lottery is unable to 593have 594personal 595contact with 597a prize599winner 600within three business days 604of the date the winner609s are posted on the website, 615the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to 636an 637alternate winner as set forth in subsection (4) above. 646If the Florida Lottery is unable to 653have 654personal 655con656tact 657with 658the first 660available 661alternate winner 663within three business days 667of the date of award of the prize, 675the 676first 677alternate winner will forfeit his or her right to claim the prize 689and the Florida Lottery will award the prize to the second 700available 701alternate. If the Florida Lottery is unable to 709have 710personal 711contact 712with 713the 714second 715alternate winner 717within three business days 721of the date of the award of prize, 729the 730second 731alternate winner will forfeit his or her right to claim the prize and the prize 746will not be awarded.
750(b) 751In the event 754a 755prizewinner does not return the required forms within the timeframe set forth in subsection (6) below, the Florida L774ottery will attempt to notify a780n 781alternate win783ner 784as described in paragraph (5)(a) above, except that 792only one 794notification attempt will be made. 799If the Florida Lottery is unable to 806have 807personal 808contact 809with 810an 811alternate winner 813within three business days of the date of award, 822the alternate winner will forfeit his or her right to claim the prize and the prize will not be awarded.
842(c) The Florida Lottery will notify each alternate in the 852Pro Football Hall of Fame 857Tailgate Party drawing by email using the contact information provided in the 869alternate’s 870registration data no later than twenty-four hours after the 879drawing. 880The email notification will notify the alternate of his or her alternate status and instruct the alternate to retain the original valid entry voucher that is required to claim a prize, until 912further 913notified by the Florida Lottery that 919he or she is 923an alternate winner of a prize or that the prizes have been fulfilled and the original valid entry voucher may be discarded.
945(9466947) How to Claim a Prize. 953To claim 955a 956prize in 958the 959Pro Football Hall of Fame 964Tailgate Party 966d967rawing, 968the 969winner 970must submit 972to 973the Florida Lottery the original valid 979entry 980voucher bearing the unique number selected in the drawing989. 990Without such voucher, the 994winner 995will forfeit his 998or her right to claim a prize. 1005Winners must submit the valid entry voucher along with 1014a completed Winner Claim Form DOL-173-2, revised 02/11, or DOL-173-2S, revised 02/11, 1026and 1027a copy of acceptable identification1032. 1033A prize winner must also submit a notarized Florida Lottery Release and Authorization form DOL-474, revised 104981050/13, or Spanish Florida Lottery Release and authorization Form DOL-474S, effective 106181062/13. 1063Forms DOL-173-2, 1065DOL-173-2S, 1066DOL-474 and DOL-474S 1069are hereby incorporated by reference and can be obtained 1078from any Lottery office, 1082from the Lottery’s website, 1086www.flalottery.com, 1087or by writing to: Florida Lottery, Customer Service, 250 Marriott Drive, Tallahassee, Florida 32399-4016. 1101The 1102required 1103form1104s 1105must be received b1109y 1110the Florida Lottery no later than 1116five 1117business 1118days 1119after the winner is notified by the Florida Lottery that he or she is a winner. 1502(8) 1503The VIP Experience prize does not include: mileage, insurance, gratuities, meals (other than those provided without charge at any event), parking fees, baggage fees, alcoholic beverages (other than those served without charge at any reception and/or party), any items not expressly specified and personal expenses such as telephone calls, valet service, laundry, incidentals and the like, as well as revision or cancellation fees which may be charged by the airlines, hotel or other supplier.

Upon receipt of a prizewinner’s required documentation, the Florida Lottery will award 1596a 1597VIP Experience 1599p1600rize and 1602will 1603provide 1604the 1605winn1606er a certificate describing the 1611VIP Experience 1613prize 1614and 1615fulfillment 1616process1617. 1618The 1619prizewinner’s 1620information 1621will be provided 1624to 1625the 1626fulfillment company, 1628MDI 1629Entertainment, LLC (“MDI”)1632. 1633MDI will contact the prizewinner w1639ithin ten business days from the date of receipt of the 1650prize1651winner1652’s 1653information 1654to make trip arrangements and shall 1660mail 1661the prizewinner a check 1665for $500 1667approximately two weeks prior to the scheduled trip departure date. 1677MDI will provide the prizewinner the appropriate IRS tax 1686f1687orm.

1807(1180821809) A 1811VIP Experience 1813p1814rize 1815is not 1817transferable 1818or assignable without the express written consent of the Lottery. If the prizewinner a1832dvises the Lottery 1835in advance of travel arrangements being made 1842that he or she is unable to take the trip portion of the VIP Experience prize, the prize winner will receive the $500 cash portion of the VIP Experience prize and may designate a proxy to use the trip portion in his or her stead and participate in the Cash Event. In such event, the 1897prize1898winner shall receive any prize won by the proxy, less applicable tax1910es, 1911and the VIP Experience prize and all prizes won by the proxy 1923in 1924the Cash Event will be taxable to the prizewinner. If the prizewinner advises the Lottery after travel arrangements have been made that he or she is unable to take the trip portion of the VIP Experience prize, the 1962prizewinner may designate a proxy to use the trip portion of the VIP Experience prize and participate in the Cash Event; however, the prizewinner shall be responsible for any revision, rebooking, transfer or cancellation fees that may be charged by the airline or other supplier.

The VIP Experience prize includes admission to a private tailgate party event for the prizewinner and up to three guests to be held on or about May 2, 2014, at the International Exposition in Cleveland, Ohio. The 2368Pro Football Hall of Fame 2373Tailgate Party 2375will be attended by approximately 100 inducted members of the Pro Football Hall of Fame (each an “HOF Member”) and a total of 200 prizewinners and their guests from 18 state lotteries. The prizewinner and his or her guests will dine with one of the HOF Members and 2423have the opportunity to play a series of tournament games with another HOF member. Prior to the 2440Pro Football Hall of Fame 2445Tailgate Party, 2447the prizewinner will have the opportunity to play a game of chance to win a cash prize of $25,000. The odds of winning a $25,000 prize are 1 in 50. If the prizewinner does not win a $25,000 prize, the prizewinner will be awarded a $1,000 prize. If the prizewinner wins a $25,000 prize, the prizewinner will have the opportunity to play a game of chance 2518during the 2520Pro Football Hall of Fame 2525Tailgate Party 2527to win an additional $1,000,000 prize. The odds of winning the $1,000,000 prize are 1 in 1,000. MDI shall mail the prizewinner a check for the total cash prize amount won at the Cash Event, less applicable Ohio state tax and federal income tax withholding, within fifteen business days after the prizewinner departs from Cleveland and shall provide each prizewinner the appropriate IRS tax form. Ohio state taxes may be recoverable from the State of Ohio. 2608Any State Owed Debt owed by the prizewinner shall also be collected as set forth in subsection (2626202627) below.
2629(2630202631) The prizewinner will be analyzed twice for State Owed Debt. The first analysis will occur at the time the VIP Experience prize is claimed. If the winner of a grand prize is identified as owing an outstanding debt to a state agency or child support collected through a court, the debt will be collected in accordance with 2689S2690ection 269124.115, 2692F.S2693. If the debt is an amount of less than the cash portion of the VIP Experience prize ($500), the non-cash portion of the prize and the cash portion of the prize less the amount owed shall be awarded. If the winner is identified as owing such a debt in an amount greater than the cash portion of the prize, the winner’s entire cash portion of the prize will be applied toward the outstanding debt as provided in 2771S2772ection 277324.115, 2774F.S., 2775and the winner will receive the remaining non-cash portion of the prize. The second analysis will occur after the prizewinner has participated in the Cash Event and the total cash won has been determined. If the winner is identified as owing an outstanding debt to a state agency or child support collected through a court, the debt will be collected in accordance with 2838S2839ection 284024.115, 2841F.S.
