53ER15. Emergency Rule For Year 2015 |

The second through sixth valid entries drawn will eac337h win a second prize of $50,000345. The seventh through sixteenth valid entries drawn will ea355ch win a third prize of $25,000363. The seventeenth through thirty-sixth valid entries drawn will eac373h win a fourth prize of $10,000381. 382The thirty-seventh through 385seventy-sixth valid entries drawn will each win a fifth prize of $5,000398. 399T400he seventy-s402eventh 403through one hundred and fifty-first 408valid entries drawn will e413ach win a sixth prize of $1,000421. The odds of winning depend on the number of qualifying entries in the Bonus Drawing.
527(5) Winner 529Notification. 530The F532lo533r534ida Lottery will attempt to notify 540the 541first 542prize winner and each 546second, third, fourth, fifth, sixth and seventh prize winner 555no later than twenty-four hours after the winner563s 564are 565posted on the Florida Lottery’s website. Notification will be made by telephone, U.S. mail or email using the contact information provided in the winner’s registration data. If the Florida Lottery is unable to hav599e personal contact with a prize605winner within two weeks of the date of the drawing, the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to the first alternate winner in accordance with subsection (6454646) above. 648If the Florida Lottery is unable to have personal contact with the first alternate winner within two weeks of the date of award of the prize, the alternate winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to a second alternate winner. This process will continue until an alternate is contacted or the Florida Lottery has exhausted the list of available alternates. If the Florida Lottery is unable to contact an alternate, t731he prize will not be awarded.
737(7386739) How to Claim a Prize. To claim the first prize or a second, third, fourth, fifth, 756sixth 757or seventh 759prize in the Bonus 763D764rawing, the winner must submit to the Florida Lottery a completed Winner Claim Form DOL 173-2, revised 9/13, or Spanish Winner Claim Form DOL 173-2S, revised 9/13 and a copy of acceptable identification as set forth in the rule of the Florida Lottery governing payment of prizes. A copy of the current prize payment rule can be obtained from the Florida Lottery, Office of the General Counsel, 250 Marriott Drive, Ta835llahassee, Florida 32399-4011. 838Forms DOL 173-2 and DOL 173-2S are hereby incorporated by reference 849and may be obtained at any Florida Lottery office, from the Florida Lottery’s website at 864flalottery.com, 865or by writing to: Florida Lottery, Customer Service, 250 Marriott Drive, Tallahassee, Florida 32399-4016. The required documentation must be received by the Florida Lottery no later than two weeks after the Florida Lottery has made personal contact with the winner. If the Florida Lottery has not received the required documentation from 916the 917first 918or a 920second, third, fourth, fifth, 924sixth 925or seventh 927prize winner by the fourteenth day after notification, the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to an alternate winner as des961cribed in subsection (4) above.

1233(1123451235) Oth1237er Restrictions and Provisions.
1241(a) All prizes are subject to the provisions of Chapter 24, F1253.1254S1255., 1256and rules promulgated thereunder. Prizes shall be paid in accordance with the rule of the Florida Lottery governing payment of prizes. A copy of the current prize payment rule can be obtained from the Florida Lottery, Office of the General Counsel, 250 Marriott Drive, Tal1301lahassee, Florida 32399-4011.
1304(b) All entries are subject to validation by the Florida Lottery and may be disqualified if eligibility requirements are not met. 1325The Florida Lottery will not enter the Webcode on a ticket received in the mai1340l or return a mailed ticket. MONOPOLY MILLIONAIRES’ CLUB tickets should not be mailed to the 1356Florida 1357Lottery unless players are contacted by the Florida Lottery and requested to do so.
1371(c) If 1373a 1374first, 1375second, 1376third, 1377fourth, fifth or sixth 1381prize winner is identified as owing an outstanding debt to a state agency or child support collected through a court, the debt will be collected in accordance with 1409S1410ection 141124.115, 1412F1413.1414S1415.
1416(d) Players must be at least 18 years of age. Persons prohibited by 1429S1430ection 143124.116, 1432F1433.1434S1435., 1436from purchasing a Florida Lottery ticket are not eligible to play.
1447(e) By 1449entering a Webcode 1452to earn entries 1455into a Second Chance Drawing and 1461automatically 1462qualifying for entry into the 1467MONOPOLY MILLIONAIRES’ CLUB Bonus Drawing, 1472a player gives his or her permission for the Florida Lottery to photograph and/or videotape and record the prizewinner with or without prior notification and to use the name, photograph, videotape, and/or recording of the prizewinner for advertising or publicity purposes without additional compensation.
1516(f) 1517The MONOPOLY MILLIONAIRES’ CLUB Bonus Drawing shall 1524be public, held in Tallahassee, Florida, and witnessed by an accountant employed by an independent certified public accounting firm. The results of 1546the 1547drawing will be available after the drawing on the Florida Lottery’s website at flalottery.com.
