    125(2) How to Enter. 129To enter a non-winning $500 A WEEK FOR LIFE, $1,000 A WEEK FOR LIFE, $2,500 A WEEK FOR LIFE, or $5,000 A WEEK FOR LIFE Scratch-Off ticket in160to 161one of the 164second chance drawings, 167players must enter on the Florida Lottery’s website at flalottery.com. On the home page of the Florida Lottery’s website, players can click on the Payday Second Chance Promotion banner and follow the directions. Players will be prompted to log-in or register. The 24-digit ticket serial number (4-digit game number and 20-digit ticket number) is located below the play area on the front of a $500 A WEEK FOR LIFE, $1,000 A WEEK FOR LIFE, $2,500 A WEEK FOR LIFE, and $5,000 A WEEK FOR LIFE Scratch-Off ticket under the latex covering. Players are to scratch off the latex covering to reveal the number and enter the entire 24-digit ticket serial number in the designated ticket entry area on the website.

Each non-winning $500 A WEEK FOR LIFE, $1,000 A WEEK FOR LIFE, $2,500 A WEEK FOR LIFE, and $5,000 A WEEK FOR LIFE Scratch-Off ticket is assigned a predetermined number of entries that corresponds to the price of the non-winning ticket, as shown in the table below.

    861(b) Grand Prize Drawing. In the grand prize drawing, a total of 873six 874valid entries will be drawn 879by the Florida Lottery using a certified random number generation process. The first valid 893entry drawn will win the grand prize of $50,000. The second through 906sixth 907valid entries drawn will be used in the order in which they were drawn to select an alternate grand prize winner in the event 931the 932grand prize cannot be awarded.

    937(7) Notification.

    939(a) The prize942winners in each second chance drawing will be posted on flalottery.com after the drawing.

    956(b) 957The F959lo960r961ida Lottery will attempt to notify 967each 968prizewinner 969no later than twenty-four hours after the winner 977is 978posted on the Florida Lottery’s website. Notification will be made by telephone, U.S. mail or email using the contact information provided in the winner’s registration data. If the Florida Lottery is unable to have personal contact with 1015the 1016grand 1017prize winner or a 1021first, 1022second or third 1025prize winner within 1028two weeks 1030of the date of the drawing, the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to the first alternate winner 1061in accordance with 1064subsection (6) above1067. 1068If the Florida Lottery is unable to have personal contact with the first alternate winner within 1084two weeks 1086of the date of 1090notification, 1091the alternate winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to a second alternate winner. This process will continue until an alternate is contacted or the Florida Lottery has exhausted the list of available alternates. If the 1139Florida 1140Lottery is unable to contact an alternate, the prize will not be awarded.

Without such ticket, the winner will forfeit his or her right to claim a prize. 1232The w1234inner must submit the valid entry ticket along with a completed Winner Claim Form DOL 173-2, revised 12519/13, 1252or Spanish Winner Claim Form DOL 173-2S, revised 12609/13 1261and a copy of acceptable identification 1267as set forth in the rule of the Florida Lottery governing payment of prizes. A copy of the current prize payment rule can be obtained from the Florida Lottery, Office of the General Counsel, 250 Marriott Drive, Tallahassee, Florida 32399-4011.  1307Forms DOL 173-2 and DOL 173-2S are hereby incorporated by reference 1318and may be obtained at any 1324Florida 1325Lottery office, from the Florida Lottery’s website at flalottery.com, or by writing to: Florida Lottery, Customer Service, 250 Marriott Drive, Tallahassee, Florida 32399-4016. The required 1350documentation 1351must be received by the Florida Lottery no later than two weeks after the Florida Lottery has made personal contact with the winner.     1816(1817d1818) Players must be at least 18 years of age. Persons 1829prohibited by 1831S1832ection 183324.116, 1834F1835.1836S1837., 1838from purchasing a Florida Lottery ticket are not eligible to play.

    1849(1850e1851) By entering 1854the 1855Payday Second Chance Promotion, 1859a player gives his or her permission for the Florida Lottery to photograph and/or videotape and record the prizewinner with or without prior notification and to use the name, photograph, videotape, and/or recording of the prizewinner for advertising or publicity purposes without additional compensation.
