58D-1. Administration Of The Alzheimer's Disease Initiative |
1(1) To be eligible 5for 6model day care services, an individual must 13be at least 18 years of age and 21have a diagnosis of Alzheimer’s disease or 28a 29related memory disorder.
32(2) 33To be eligible to receive all other services funded under the 44Alzheimer’s disease 46Initiative that are included in the provider’s contract with the department or the area agency on aging, an i65ndividual 66must be 18 years of age or older and have a diagnosis of 79Alzheimer’s disease 81or related memory disorder, 85or be suspected of having Alzheimer’s disease or 93a 94related memory disorder97.
98(3) The caregivers of individuals receiving services under the A108lzheimer’s Disease Initiative 111are eligible to receive training and related support services to assist them in caring for the person with 129ADRD130.
