59G-4.340. Visual Aid Services

Effective on Sunday, November 15, 2015

1(1) This rule applies to all providers of visual aid services who are enrolled in or registered with the Florida Medicaid program.

23(2) All providers of visual aid services must be in compliance with the provisions of the Florida Medicaid Visual Aid Services Coverage Policy, November 2015, incorporated by reference.
