5J-20. Liquefied Petroleum Gas Inspection |
1(1) Documentation of employee training, as required in NFPA 58, incorporated in rule 145J-20.002, 15F.A.C., shall include the following:
20(a) Employee’s name and signature;
25(b) Employee’s primary job responsibilities and duties;
32(c) Date of completed employee training;
38(d) Description, copy or location of the materials used to conduct the training, including the topics covered by the training;
58(e) Name, address and phone number of the person or organization conducting the training;
72(f) Signature of employee supervisor or trainer verifying training and evaluation.
83(2) Documentation shall be maintained at the employee’s place of business and shall be available for inspection by the department upon request.
