60DD-5. Communications Procurement Of Customer-Owned Equipment |
1(1) Agencies shall request approval from the State Technology Office for the purchase or lease of all communications equipment and services when the cost exceeds the dollar amount set forth as Category Two in Section 36287.017, 37Florida Statutes. Such cost shall be determined by including all recurring and non-recurring charges for the first year. The fair market value of any loan or gift of communications equipment or services shall be included in the cost.
75(2) Agencies shall request approval from the State Technology Office for purchases and leases by submitting Form STO-COM-9008, Communications Purchase or Lease Authorization Form (“CPLA”), effective September 23, 2004, which is hereby incorporated by reference. 127The requesting agency shall furnish to the State Technology Office upon request any supplementary documentation necessary to properly evaluate the agency request.
