60L-39.005. Application Procedures (Repealed)  


Effective on Sunday, December 27, 2015
  • 1(1) A Completed Application shall be submitted no later than March 1 of each year for the Campaign Cycle beginning that year.

    23(a) Electronic applications shall be submitted to the electronic address specified by the Department on the application form by 11:59 p.m. (Eastern Time), on March 1.

    49(b) In the event the application form and supporting documentation are submitted as a paper package, the submission must be postmarked by March 1.

    73(c) An umbrella group may submit applications on behalf of its members.

    85(d) Each charitable organization shall document on Question 12 of the application, 97administrative expenses, fundraising expenses and total revenue from the organization’s most recently filed IRS Form 990, which must cover a fiscal period ending not more than 24 months prior to March 1. Charitable Organizations that are not required to file an IRS Form 990 or that file an IRS Form 990 EZ or an IRS Form 990 PF shall provide analogous figures in Question 12, in accordance with the instructions on the Form DMS-ADM-100, rev. 12/2015.

    173(2) The Statewide Steering Committee shall only consider complete applications for inclusion in the Campaign. Incomplete applications shall be deemed denied without further action from the Statewide Steering Committee.
