60P-9.004. Effective Date of Coverage

  • 1The effective date of coverage for an employee under the Plan shall be as follows:

    16(1) The date of an employee’s appointment to an eligible position; unless the employee is disabled or under a physician’s care on account of sickness or injury and not actively at work on that date, the effective date will be the date the employee resumes active work.

    63(2) An employee may elect to refuse coverage by signing a refusal statement and submitting it to his or her personnel office.

    85(3) The effective date of an employee who has previously refused coverage will be the first day of the month following the receipt of the employee’s written request to enroll; unless the employee is disabled or under a physician’s care on account of sickness or injury and not actively at work on that date, the effective date will be the date the employee resumes active work.
