61A-10.085. Duplicate License Request

Effective on Tuesday, September 2, 2008

1Any permitee desiring to replace a lost or destroyed permit, may make a request to the Division for the issuance of a duplicate permit. This request shall be in the form of an affidavit advising that the permit has been lost or destroyed.
