61D-14.059. Slot Machine Licensee Personnel

  • 1(1) Each slot machine licensee shall maintain a level of staffing, supervision, and delineation of responsibilities of the organizational structure that ensures the proper operations as provided in the internal control procedures.

    33(2) The slot machine licensee’s internal controls shall provide for a pre-employment screening process for each prospective employee, including the following:

    54(a) A completed employment application;

    59(b) Documentation that prospective employees for supervisory, managerial or surveillance positions have skills, training and experience for the position; and,

    79(c) A reviewed credit report.

    84(3) The slot licensee shall maintain a personnel file for each employee that shall contain without limitation the following:

    103(a) The original employment application required by the slot licensee;

    113(b) The initial credit report and all subsequent credit reports that shall be obtained at least every 36 months;

    132(c) A copy of all occupational licenses issued by the division during employment;

    145(d) A current detailed position description that includes access rights granted to the employee relating to secure areas, keys, or information;

    166(e) A chronological log of all positions held by the employee indicating the effective and termination date of each position;

    186(f) All performance evaluations conducted by the slot licensee;

    195(g) Documentation relating to performance issues such as promotions, demotions, reprimands, or separations;

    208(h) 209Signed drug-free workplace agreement based upon the licensee’s established drug-testing program which shall include, at a minimum, the following elements:

    2291. A generalized list of substances for which licensees may be tested,

    2412. A timeline of when drug testing may occur; and,

    2513. Disciplinary actions the slot machine licensee may take in the event of a positive test result.

    268(i) All federal and state income tax returns filed by the employee for the previous three years, if the employee is identified in paragraph 29261D-14.002(1)(e), 293F.A.C., or is required to hold a professional occupational license.
