61G10-12. Fee Schedule |
1(1) The application fee for licensure by endorsement shall be one hundred and fifty dollars ($150.00).
17(2) The fee for initial licensure is one hundred and twelve dollars and fifty cents ($33112.5034).
35(3) The fee for biennial renewal of a certificate of registration shall be two hundred twenty-five dollars ($225.00).
53(4) The fee for renewing an inactive license shall be fifty dollars ($50.00).
66(5) The application fee for a temporary license shall be four hundred and fifty dollars ($450.00).
82(6) There shall be a five dollar ($5.00) fee collected by the Department upon initial licensure and licensure renewal for the purpose of combating unlicensed activity.
108(7) The delinquency fee shall be one twenty-five dollars ($25.00).
118(8) A processing fee of fifty dollars ($50.00) will be charged when a licensee changes status at any time during the biennium, other than the normal biennial renewal period.
147(9) The application fee for reinstatement of a null and void license is $450.00.
