61G5-25.004. Null and Void Status

1The Null and Void status licensee who applies for active or inactive status shall apply to the Department by submitting Form DBPR COSMO 7 Application for License/Registration from Null and Void (Expired License/Registration), incorporated by Rule 3761-35.011, 38F.A.C.
