61J1-8.005. Revocation

  • 1(1) Pursuant to Section 5455.227(5), F.S., 7revocation of a registration, license or certification is permanent except for the following violation: Rule 2261J1-4.005, 23F.A.C. – filing an application for renewal of a registration, license or certification when the individual had not complied with the provisions of Rule 4761J1-4.003, 48F.A.C.

    49(2) An individual whose registration, license or certification has been revoked for the above listed violation may not apply for registration for a period of five (5) years after the date of filing of the final order revoking the registration, license or certification. The applicant must meet all the requirements for initial registration as a registrant. Certification, licensure or registration will be based upon the criteria listed in Sections 118475.615 119and 120475.624 121or 122475.6235 123and 124475.6245, F.S., 126as applicable.
