61N-2.010. Application for Medical Oxygen Retail Establishment Permit

A medical oxygen retail establishment permit is required for an entity that is located in the state and that sells or delivers medical oxygen directly to patients in this state. A person located in this state, other than a pharmacy licensed under Chapter 465, F.S., prior to engaging in activity for which a medical oxygen retail establishment permit is required, must file with the department a completed application on form number DBPR-DDC-223, Application for Permit as a Medical Oxygen Retail Establishment, effective April 2016, adopted and incorporated herein by reference and comply with all the requirements for permitting in Chapter 499, F.S., and Title 61N, F.A.C.
