1(1) All facilities and programs shall implement a method of identifying and solving potential and actual problems in health care delivery to committed youth.
25(2) Meetings shall be held no less than quarterly, whereby all disciplines that provide or oversee the provision of physical and mental health care, programming/operations and behavior management are represented. Additional meetings shall be held as needed when an adverse or sentinel event occurs or the potential for such an event is recognized.
78(3) Simple Root Cause Analysis or another problem-solving methodology shall be conducted for review of actual adverse or sentinel events.
