63N-1. Service Delivery |
1(1) 2Comprehensive Mental Health Evaluations shall be administered by a Licensed Mental Health Professional or a Mental Health Clinical Staff Person working under the direct supervision of a Licensed Mental Health Professional.
33(2) Comprehensive Mental Health Evaluations shall reflect consideration of the following:
44(a) Identifying Information;
47(b) Reason for Evaluation;
51(c) Relevant Background Information, including home environment and family functioning; history of physical abuse, sexual abuse, neglect, witnessing violence and other forms of trauma; behavioral functioning; physical health mental health and substance abuse history and educational functioning.
88(d) Behavioral Observations;
91(e) Mental Status Examination;
95(f) Interview or Procedures Administered;
100(g) Discussion of Findings;
104(h) Diagnostic Impression/Formulation including 108DSM diagnoses; and
111(i) Recommendations.
113(3) Comprehensive Mental Health Evaluations shall be completed within 30 days of referral. However, if screening, staff observations or other information indicates the youth has a mental health problem which poses a safety risk to himself/herself or others, completion of the Comprehensive Mental Health Evaluation must be expedited based upon the urgency of the youth’s symptoms as determined by a Licensed Mental Health Professional. When the Comprehensive Mental Health Evaluation indicates the youth is in need of treatment, the youth shall receive an Initial or Individualized Mental Health Treatment Plan and appropriate treatment services.
207(4) Updated Comprehensive Mental Health Evaluation.
213(a) When the youth’s file contains a Comprehensive Mental Health Evaluation completed within twelve months of the youth’s admission, the previous corresponding Comprehensive Mental Health Evaluation may be utilized to conduct an updated Comprehensive Mental Health Evaluation.
250(b) The updated Comprehensive Mental Health Evaluation must be administered by a Licensed Mental Health Professional or a Mental Health Clinical Staff Person working under the direct supervision of a Licensed Mental Health Professional 284in accordance with the provisions below:
2901. The updated Comprehensive Mental Health Evaluation must be clearly identified as such and must be attached to the previous comprehensive evaluation which is being updated.
3162. The updated Comprehensive Mental Health Evaluation must provide any new or additional information applicable to each area specified in subsection (2) above, based upon current information provided by the youth, his or her family/legal guardians and the youth’s records.
