63N-1.0086. Mental Health and Substance Abuse Transition/Discharge Planning

  • 1(1) Mental Health Transition/Discharge Planning.

    6(a) During the final phase of mental health treatment, the Mental Health Clinical Staff Person, treatment team and youth shall establish a transition/discharge plan whereby improvements made during mental health treatment will be maintained upon the youth’s movement from one facility to another, or return to the community.

    54(b) A transition/discharge plan shall be documented on the Mental Health/Substance Abuse Treatment Discharge Plan Form (MHSA 011, October 2014) which is incorporated by reference and is available at 83http://www.flrules.org/Gateway/reference.asp?No=Ref-05371 85or may be obtained by contacting: DJJ, Office of Health Services, 2737 Centerview Drive, Tallahassee, FL 32399.

    1021. The mental health information contained in the Mental Health/Substance Abuse Treatment Discharge Plan shall be discussed with the youth, parent/legal guardian (when available) and Juvenile Probation Officer prior to the youth’s release from the facility or program.

    1402. A copy of the Mental Health/Substance Abuse Treatment Discharge Plan Form (MHSA 011) will be provided to the youth, the youth’s assigned Juvenile Probation Officer, and also to the parent/legal guardian when the youth’s written consent for release of substance abuse information to the parent/guardian has been obtained in accordance with consent provisions in Rule 19663N-1.015, 197F.A.C.

    198(c) Transition planning for youths on Suicide Risk Alert/Suicide Precautions immediately prior to discharge to the community shall include notification of the youth’s parent/legal guardian and Juvenile Probation Officer in accordance with Rule 23163N-1.0097, 232F.A.C.

    233(d) Transition planning for youths on Suicide Risk Alert/Suicide Precautions immediately prior to transfer to another DJJ facility or program shall include notification of the facility superintendent/program director where the youth is to be transferred in accordance with Rule 27263N-1.0097, 273F.A.C.

    274(2) Substance Abuse Transition/Discharge Planning.

    279(a) During the final phase of substance abuse treatment, the Licensed Qualified Professional or 293Substance Abuse Clinical Staff Person 298the treatment team and youth shall establish a transition/discharge plan whereby improvements made during substance abuse treatment will be maintained upon the youth’s movement from one facility to another, or return to the community.

    332(b) The transition/discharge plan shall be documented on the Mental Health/Substance Abuse Treatment Discharge Plan (form MHSA 011).

    3501. The substance abuse information contained in the Mental Health/Substance Abuse Treatment Discharge Plan shall be discussed with the youth, parent/legal guardian (when available) and Juvenile Probation Officer prior to the youth’s release from the facility or program.

    3882. A copy of the Mental Health/Substance Abuse Treatment Discharge Plan Form (MHSA 011) will be provided to the youth, the youth’s assigned Juvenile Probation Officer and also to the parent/legal guardian when the youth’s written consent for release of substance abuse information to the parent/guardian has been obtained in accordance with consent provisions in Rule 44463N-1.0097, 445F.A.C.
