    17(a) Section 19456.062, F.S., 21entitled “Advertisement by Health Care Provider of Free or Discounted Services; Required Statement.”

    34(b) Section 36456.057, F.S., 38entitled “Ownership and control of patient records; report or copies of records to be furnished.”

    53(c) Section 55463.011, F.S., 57entitled “Exhibition of License.”

    61(d) Subsection (1), paragraphs (2)(a) and (b) of section 70463.012, F.S., 72entitled “Prescriptions; Filing; Release; Duplication.”

    77(e) Subsection 79463.0135(11), F.S., 81entitled “Standards of Practice,” if the violation is of a technical nature not related to patient care.

    99(f) Paragraphs (1)(d) and (e) of section 106463.014, F.S., 108entitled “Certain Acts Prohibited.”

    112(g) For the first violation of subsection 119463.0141, F.S., 121entitled “Reports of adverse incidents in the practice of optometry.”

    196(m) Subsections (1), (2) and (5) of section 204465.0276, F.S., 206entitled “Dispensing Practitioner,” if the violation is of a technical nature not related to patient care.

    309(e) For a second or subsequent violation of Section 318463.0141, F.S., 320entitled “Reports of adverse incidents in the practice of optometry.

    518(p) Section 520465.0276(2)(d)1., F.S., 522dispensing a controlled substance to a person not known to the dispenser without requiring valid photographic identification or other verification of identity.

    544(q) Section 546893.055(3), F.S., 548failure to report information regarding dispensed controlled substances to the 558Prescription Drug Monitoring Program Controlled Substance Dispensing Information Electronic System568.

    569(r) Section 571893.0551(6), F.S., 573failure to maintain confidentiality of information obtained from the Prescription Drug Monitoring Program Controlled Substance Dispensing Information Electronic System.

    592(s) Section 594893.055(8), F.S., 596second or subsequent failure to consult the Prescription Drug Monitoring Program Controlled Substance Dispensing Information Electronic System prior to prescribing or dispensing a schedule IV or schedule V opioid controlled substance.

    627(3) For any offense which is not specified above or in subsection (1) or (2) of Rule 64464B13-15.006, 645F.A.C., the Board will apply the guideline penalty based on the offense listed which is most comparable to the offense charged.
