64B16-26. Pharmacists Licensure |
1(1) A pharmacist licensee may elect:
7(a) At the time of license renewal to place the license on inactive status by submitting a written request with the board for inactive status and submitting the inactive status renewal fee of $200 plus a $5 unlicensed activity fee pursuant to Section 50456.065(3), F.S.
52(b) At the time of license renewal, if the license is inactive, to continue the license on inactive status by submitting a written request with the board for inactive status and submitting the inactive status renewal fee of $200 plus a $5 unlicensed activity fee pursuant to Section 100456.065(3), F.S.
102(c) At the time of license renewal to change the inactive status license to active status, provided the licensee meets the continuing education requirements of Rule 12864B16-26.103, 129F.A.C., for each biennium the license was on inactive status, submits the reactivation fee of $70, and the current active renewal fee set forth in Rule 15564B16-26.1001, 156F.A.C.
157(d) At a time other than license renewal to change the inactive status license to active status, provided the licensee meets the continuing education requirements of Rule 18464B16-26.103, 185F.A.C., for each biennium the license was on inactive status and submits the reactivation fee of $70, a change of status fee of $25 and the difference between the inactive status renewal fee and the active status renewal fee, if any exists.
227(2) A consultant pharmacist licensee may elect:
234(a) At the time of license renewal to place the license on inactive status by submitting a written request with the board for inactive status and submitting the inactive status renewal fee of $100 plus a $5 unlicensed activity fee pursuant to Section 277456.065(3), F.S.
279(b) At the time of license renewal, if the consultant pharmacist license is inactive, to continue the license on inactive status by submitting a written request with the board for inactive status and submitting the inactive status renewal fee of $100 plus a $5 unlicensed activity fee pursuant to Section 329456.065(3), F.S.
331(c) At the time of license renewal to change the inactive status consultant pharmacist license to active status, provided the consultant pharmacist licensee meets the continuing education requirements of subsection 36164B16-26.103(2), 362F.A.C., for each biennium the license was on inactive status and by submitting a reactivation fee of $25, and the active consultant pharmacist renewal fee set forth in Rule 39164B16-26.1003, 392F.A.C.
393(d) At a time other than license renewal to change the inactive status license to active status, provided the licensee meets the continuing education requirements of Rule 42064B16-26.103, 421F.A.C., for each biennium the license was on inactive status, and submits the reactivation fee of $25, a change of status fee of $25 and the difference between the inactive status renewal fee and the active status renewal fee, if any exists.
463(3) A nuclear pharmacist licensee may elect:
470(a) At the time of license renewal to place the nuclear pharmacist license on inactive status by submitting a written request with the board for inactive status and submitting the inactive status renewal fee of $100 plus a $5 unlicensed activity fee pursuant to Section 515456.065(3), F.S.
517(b) At the time of license renewal, if the nuclear pharmacist license is inactive, to continue the license on inactive status by submitting a written request with the board for inactive status and submitting the inactive status renewal fee of $100 plus a $5 unlicensed activity fee pursuant to Section 567456.065(3), F.S.
569(c) At the time of license renewal to change the inactive status license to active status, provided the nuclear pharmacist meets the continuing education requirements of Rule 59664B16-26.304, 597F.A.C., for each biennium the license was on inactive status, and by submitting a reactivation fee of $50, and the active nuclear license renewal fee set forth in Rule 62664B16-26.1003, 627F.A.C.
628(d) At a time other than license renewal to change the inactive status license to active status, provided the nuclear pharmacist licensee meets the continuing education requirements of Rule 65764B16-26.304, 658F.A.C., for each biennium the license was on inactive status and by submitting a reactivation fee of $50, a change of status fee of $25 and the difference between the inactive status renewal fee and the active status renewal fee, if any exists.
701(4) A registered pharmacy technician may elect:
708(a) At the time of renewal to place the registered pharmacy technician registration on inactive status by submitting a written request with the board for inactive status and submitting the inactive status renewal fee of $50 plus a $5 unlicensed activity fee pursuant to Section 753456.065(3), F.S.
755(b) At the time of renewal, if the registered pharmacy technician registration is inactive, to continue the registration on inactive status by submitting a written request with the board for inactive status and submitting the inactive status renewal fee of $50 plus a $5 unlicensed activity fee pursuant to Section 805456.065(3), F.S.
807(c) At the time of renewal to change the inactive status registration to active status, provided the registered pharmacy technician meets the continuing education requirements of Rule 83464B16-26.103, 835F.A.C., for each biennium the registration was on inactive status, and by submitting a reactivation fee of $50, and the active registration fee set forth in Rule 86264B16-26.1003, 863F.A.C.
864(d) At a time other than renewal to change the inactive status registration to active status, provided the registered pharmacy technician meets the continuing education requirements of Rule 89264B16-26.103, 893F.A.C., for each biennium the registration was on inactive status and by submitting a reactivation fee of $50, a change of status fee of $25 and the difference between the inactive status renewal fee and the active status renewal fee, if any exists.
