64B16-26.1021. Delinquent License Reversion; Reinstatement; Fees  


Effective on Tuesday, October 27, 2009
  • 1(1) An active or inactive license that is not renewed by midnight of the expiration date of the license shall automatically revert to delinquent status.

    26(2) A pharmacist 29may request that a delinquent license be reinstated to active or inactive status, provided the licensee meets the continuing education requirements of Rule 5264B16-26.103, 53F.A.C., for each biennium the license was on inactive status, and by submitting a reactivation fee of $7110720 plus the current fee for an active status or inactive status license set forth in Rule 8964B16-26.1003 90or 9164B16-26.1004, 92F.A.C.

    93(3) A consultant pharmacist may request that a delinquent consultant pharmacist license be reinstated to an active or inactive status by submitting a delinquent fee of $120100 121plus the current fee for an active or inactive status consultant pharmacist license set forth in Rule 13864B16-26.1003 139or 14064B16-26.1004, 141F.A.C.

    142(4) A nuclear pharmacist may request that a delinquent nuclear pharmacist license be reinstated to an active or inactive license status by submitting a delinquent fee of $100 plus the current fee for an active or inactive nuclear pharmacist license set forth in Rule 18664B16-26.1003 187or 18864B16-26.1004, 189F.A.C.

    190(5) A registered pharmacy technician may request that a delinquent registered pharmacy technician registration be reinstated to an active or inactive status provided the registered pharmacy technician meets the continuing education requirements of Rule 22464B16-26.103, 225F.A.C., for each biennium the registration was on inactive status, and by submitting a reactivation fee of $25 plus the current fee for an active or inactive status registered pharmacy technician registration set forth in Rule 26164B16-26.1003 262or 26364B16-26.1004, 264F.A.C.

    265(2666267) A license in delinquent status that is not renewed prior to midnight of the expiration date of the current licensure cycle shall be rendered null without any further action by the Department. Any subsequent license shall be the result of applying for and meeting all requirements imposed on an applicant for new licensure.
