64B5-15.002. Application Fees

Effective on Monday, April 27, 2009

1The application fee for licensure as a dentist shall be one hundred dollars ($100.00), and the application fee for licensure as a dental hygienist shall be fifty dollars ($50.00). The application fee for a Health Access Dental License shall be one hundred dollars ($100.00).
