64D-3.034. Cancer Reporting

  • 1(1) Reporting Requirements:

    4(a) Each facility and laboratory licensed under Chapters 395 and 483, and Section 17408.07(20), F.S., 19respectively and practitioners licensed under Chapters 458, 459, 464, F.S., are required to report to the Florida Cancer Data System as required by Section 43385.202, F.S., 45within six (6) months of each diagnosis and within six (6) months of the date of each treatment.

    63(b) Each facility shall submit each cancer case report electronically. Those facilities with fewer than 35 cancers annually requiring abstracting may submit paper copies or portions of the medical record, provided the copies contain all of the required information as per paragraph (1)(c).

    106(c) The data items, coding schemes, definitions, record layouts and reporting procedures are to follow the guidance provided in the Florida Cancer Data System Data Acquisition Manual (2005, or current year edition), incorporated by reference, available at:  143http://fcds.med.miami.e144d145u/inc/downloads.shtml146.

    147(2) Not withstanding subsection (1), each facility, center and laboratory that reports cancer cases to the Florida Cancer Data System shall make its records available for on-site review by the Department or its authorized representatives.
