64F-18. Insulin Distribution Program |
1(1) “Bona fide resident” means a person living in Florida with the intent to remain.
16(2) “Current prescription” means a prescription written by a licensed health care practitioner who is authorized by law to prescribe medicine within 3 months of application and effective for up to 12 months after it is written. The prescription must be written by a licensed health care practitioner authorized by law to prescribe medicine and include the following information:
75(a) Person’s name (printed or typed);
81(b) Person’s date of birth;
86(c) Physician’s state license number;
91(d) Physician’s name (printed or typed);
97(e) Physician’s phone number;
101(f) Date of prescription;
105(g) Type of insulin to be issued;
112(h) Medication dosage;
115(i) Amount of medication to be issued at each visit – up to a three month supply;
132(j) Whether and how many refills are allowed.
140(3) “Designated agent” means any pharmacy that has entered into a written agreement with a county health department to provide insulin to approved insulin distribution program participants.
167(4) “Family” means one or more persons living in one dwelling place who are related by blood, marriage, law or conception. A single adult, over 18, living with relatives is considered to be a separate family.
203(5) “Federal poverty guidelines” mean the poverty guidelines defined by subsection 21464F-16.001(6), 215F.A.C.
216(6) “Gross family income” means the sum of income available to a family at the time of application. Gross family income is based on all income to be earned or received or anticipated to be earned or received in a current month. Gross family income does not include Supplemental Security Income (SSI) or any income received by the SSI eligible individual(s) and any income received by the minor sibling(s) of the eligible individual(s). Gross family income includes the following:
295(a) Wages and salary;
299(b) Child support;
302(c) Alimony;
304(d) Unemployment compensation;
307(e) Worker’s compensation for lost income;
313(f) Veteran’s pension;
316(g) Social Security;
319(h) Pension or annuities;
323(i) Dividends, interest on savings, stocks or bonds;
331(j) Income from estates or trusts;
337(k) Net rental income or royalties;
343(l) Net income from self-employment;
348(m) Contributions;
350(n) Public assistance or welfare payments;
356(o) Cash amounts received or withdrawn from any source including savings, investments, trust accounts and other resources which are readily available to the family;
380(p) Other case income.
384(7) “Net family income” means gross family income minus the standard work-related, child care and child support expenses or deductions as used in determining presumptive eligibility for Medicaid.
412(8) “Self declaration” means a written statement regarding assets, insurance coverage, income, family size and residency made by a person applying for the insulin distribution program. Self-declaration does not include any documentation other than the signature of the person making the statement. The self-declaration statement that the department requires under this chapter shall include a signed acknowledgement by the applicant that the statement is true at the time it is made and that the applicant understands that the county health department shall have the option of verifying the information provided.
502(9) “Verification” means to confirm the accuracy of information through sources other than the self-declaration statement of the individual that originally supplied the information.
