64B1-7.002. Requirements for Reactivation of an Inactive License

  • 1(1) An inactive license shall be reactivated upon payment of the reactivation fee set forth in subsection 1864B1-2.010(3), 19F.A.C., and demonstration that the licensee completed the continuing education requirements as set forth in Rule 3564B1-7.0015, 36F.A.C., for each biennium in which the license was inactive.

    46(2) However, any licensee whose license has been inactive for more than two consecutive biennial licensure cycles and who has not practiced for two out of the previous four years in another jurisdiction shall be required to appear before the Board and establish the ability to practice with the care and skill sufficient to protect the health, safety, and welfare of the public. At the time of such appearance, the licensee must:

    118(a) Show compliance with subsection (1) above;

    125(b) Account for any activities related to the practice of acupuncture in this or any other jurisdiction during the period that the license was inactive and establish an absence of malpractice or disciplinary actions pending in any jurisdiction;

    163(c) Prove compliance with the financial responsibility requirements of Section 173456.048, F.S., 175and Rule 17764B1-12.001, 178F.A.C.;

    179(d) Prove compliance with Section 184456.033, F.S., 186and Rule 18864B1-7.0015, 189F.A.C.

    190(3) The Department shall not reactivate the certificate of any certificate holder who has:

    204(a) Committed any act or offense in this or any other jurisdiction which would constitute the basis for disciplining a certificate holder pursuant to Section 229457.109, F.S.

    231(b) Failed to comply with the financial responsibility requirements of Section 242456.048, F.S., 244and Rule 24664B1-12.001, 247F.A.C.

    248(c) Failed to comply with the provisions of Section 257456.033, F.S., 259and Rule 26164B1-7.0015, 262F.A.C.
