To obtain a optometric faculty certificate pursuant to Section 463.0057, F.S., the applicant must submit an Initial Optometry Faculty Certificate Application, form number DH-MQA 1134, (07/2020) which is hereby incorporated by reference and can be obtained from the board office or at: http://www.floridaasoptometry.gov, or http://www.flrules.org/Gateway/reference.asp?No=Ref-12453, and the fees set forth in Rule 64B13-6.001, F.A.C.
