64B14-4.003. Documentation of Eligibility for Licensure

  • 1(1) In order to establish eligibility for licensure as an orthotist, prosthetist, orthotic fitter, orthotic fitter assistant, or pedorthist under section 22468.803(5)(a), 23(b), (c), (d) or (e), F.S., the applicant must submit an Application for Licensure, form DH-MQA 1132, 08/2017, herein incorporated by reference, which is available at 49https://www.flrules.org/Gateway/reference.asp?No=Ref-09988, 51or at the Board’s 55website 56http://floridasorthotistsprosthetists.gov/, 57and provide:

    59(a) An official transcript from the regionally accredited institution at which the applicant’s Baccalaureate or 74post-graduate Bachelor of Science degree was awarded, indicating the degree awarded.

    85(b) If the applicant’s degree was not awarded in orthotics and prosthetics, documentation from the CAAHEP approved program certifying completion of an orthotics or prosthetics program, as applicable.

    113(c) If the applicant completed a residency program certified by NCOPE, an original letter from the head of the residency program attesting to successful completion of the program.

    141(d)1. If the applicant completed an internship, a patient log signed by the applicant’s qualified supervisor(s), including the types of patients treated, the types of treatment provided, and the number of hours worked, that demonstrates compliance with rule 17964B14-4.100, 180F.A.C.

    1812. The Board shall accept written documentation from ABC establishing the completion of an internship. Such documentation must be submitted to the Board office directly from ABC and must demonstrate the applicant’s internship was in compliance with rule 21964B14-4.100, 220F.A.C.

    221(e) Documentation of completion of the mandatory courses.

    229(f) Sections 231468.803(4)(a)2., 232(4)(b)2., (5)(a)2., and (5)(b)2., F.S., require that an applicant for examination or license must first complete a 1 year qualified experience internship or residency. Experience gained in Florida, either through a residency or internship, is deemed “qualified” beginning only after the issuance date of a valid Registration to practice in Florida as either an intern or resident.

    289(2) In order to establish eligibility for licensure as an orthotic fitter under section 303468.803(5)(c), F.S., 305the applicant must provide at a minimum:

    312(a) A high school diploma or a certified GED certificate.

    322(b) An original or certified copy of a certificate of completion of the Trulife Healthcare or the Surgical Appliance Industries orthotics course and examination.

    346(c) Verification of Employment Form(s) demonstrating two years supervised experience in orthotics that meets the requirements of paragraph 36464B14-4.110(1)(b), 365F.A.C., signed by the applicant’s qualified supervisor(s).

    372(d) Certification of completion of an 8 hour course in casting custom-molded shoes.

    385(e) Documentation of completion of the mandatory courses.

    393(3) In order to establish eligibility for licensure as an orthotic fitter assistant under section 408468.803(5)(d), F.S., 410the applicant must provide at a minimum:

    417(a) A high school diploma or a certified GED certificate.

    427(b) An original or certified copy of a certificate of completion of the Trulife Healthcare or the Surgical Appliance Industries orthotics course and examination.

    451(c) Certification of completion of an 8 hour course in casting custom-molded shoes.

    464(d) Documentation of completion of the mandatory courses.

    472(4) In order to establish eligibility for licensure as a pedorthist under section 485468.803(5)(e), F.S., 487the applicant must provide at a minimum:

    494(a) A high school diploma or a certified GED certificate.

    504(b) An original letter from the head of a training program(s) approved by the Board for Certification in Pedorthics, (BCP), or the American Board for Certification (ABC), attesting to the inclusion of training as provided in paragraph 54164B14-4.110(3)(a), 542F.A.C.

    543(c) A patient log signed by the applicant’s qualified supervisor(s), including the types of patients treated, the types of treatment provided, and the number of hours worked.

    570(d) Documentation of completion of the mandatory courses.
