1Applications for Temporary Certificate for Active Duty Military and Veterans Practicing in Areas of Critical Need must include a completed application form and appropriate licensure fee as set forth in Section 32459.00761, F.S., 34and Rule 3664B15-10.002, 37F.A.C. The instructions and application form, DH-MQA 5002, (Revised 5/22), entitled “Osteopathic Physician Application for Temporary Certificate for Active Duty Military and Veterans Practicing in Areas of Critical Need” is hereby incorporated by reference, and may be obtained from 76http://www.flrules.org/Gateway/reference.asp?No=Ref-14613, 78from79the Board of Osteopathic Medicine, 4052 Bald Cypress Way, Bin #C06, Tallahassee, Florida 32399-3256, or from the website at: https://floridasosteopathicmedicine.gov/. Such application and fee shall expire one year from the date on which the application is initially received by the Board. After a period of one year a new application and fee must be submitted.
