1Applicants for CPC shall submit an application using Form DH5059-MQA (eff. 08/20), “Application for Pharmacist Collaborative Practice Certification” that is hereby incorporated by reference and available at 28http://www.flrules.org/Gateway/reference.asp?No=Ref-12225 30or http://floridapharmacy.gov Applicants for certification shall meet and comply with all requirements in Section 46455.1865, F.S.
