1(1) The specially designated area of need for those persons licensed pursuant to Section 15461.018, 16Florida Statutes, as created in 1996, is Dade County, Florida.
26(2) For licensure pursuant to Section 32461.018, 33Florida Statutes, the fee for initial application, licensure, and renewal is $100.
45(3) For licensure pursuant to Section 51461.018, 52Florida Statutes, the fee for renewal of inactive status is $50.
63(4) For licensure pursuant to Section 69461.018, 70Florida Statutes, the fee for renewal of delinquent status is $50.
81(5) For licensure pursuant to Section 87461.018, 88Florida Statutes, the fee for renewal or reactivation is $50.
98(6) For persons licensed pursuant to Section 105461.018, 106Florida Statutes, the fee for unlicensed activity is $5 which is imposed by the Department and shall be in addition to the other licensure fees.
131(7) For licensees licensed pursuant to Section 138461.018, 139Florida Statutes, proof of financial responsibility shall be demonstrated by a letter of credit for $1,000.
