64B19-12.006. Reactivation Fee and Change of Status Fee

Effective on Tuesday, February 8, 2022

1The fee for reactivation of an inactive or retired status license is $50.00. Upon any change of status, including the election of retired status, a $50.00 change of status fee shall be charged. The change of status fee for a limited license shall be $25.00. Such fee(s) shall be in addition to the biennial licensure fee, if any, as prescribed in rule 6364B19-12.005, 64F.A.C.
