64B19-14.001. Request for Retired Status

Effective on Sunday, July 16, 2006

1A licensee with an active or inactive license may choose retired status by submitting a written request to the Board and remitting the retired status fee set out in Rule 3164B19-12.013, 32F.A.C, and, if applicable, the change of status fee set out in Rule 4564B19-12.006, 46F.A.C.
