64B2-12.014. Supervising Physician Application Fee

Effective on Sunday, May 24, 2009

1(1) The fee for the initial application to supervise a certified chiropractic physician’s assistant shall be one hundred dollars ($100.00).

21(2) The fee for the 26biennial renewal certification 29to supervise a certified chiropractic physician’s assistant shall be one hundred dollars ($100.00).
