64B20-9.003. Place of Practice

Effective on Thursday, May 26, 2005

1It shall be the duty of each licensee or certificateholder to provide to the Department written notification of his or her current mailing address and place of practice within 30 days of the change. The term “place of practice” means the address of the primary location where the licensee or certificateholder practices.
