64B24-3. Fees |
1(1) The application fee shall be $200.00.
8(2) The 4-month prelicensure course application fee shall be $100.00.
18(3) The examination fee shall be paid to the examination vendor and refunded if the applicant is ineligible to sit for the examination.
41(4) The endorsement fee shall be $250.00.
48(5) The intitial licensure fee shall be $500.00, whether by examination or endorsement.
61(6) The temporary certificate fee shall be $50.00 in addition to the fee required for licensure.
77(7) The active biennial renewal fee shall be $500.00.
86(8) The delinquent fee shall be $75.00.
93(9) The reactivation fee shall be $500.00.
100(10) The duplicate license fee shall be $25.00.
108(11) The continuing education provider application fee shall be $250.00.
118(12) The change of status fee shall be $75.00.
127(13) The inactive status renewal fee shall be $500.00.
136(14) The retired status fee shall be $50.00.
144(15) The unlicensed activity fee shall be $5.00.
