Effective on Tuesday, March 17, 2009

1Each licensed midwife shall provide Council staff with either written or electronic notification of 15one current mailing address. The current mailing address and place of practice is defined as an address acceptable to the United States postal service where the licensed midwife shall be served with notices pertaining to licensure.
