1(1) The non-refundable application fee is $50.
8(2) The fee for initial certification is $150.
16(3) The non-refundable application fee for licensure by endorsement is $150.
27(4) The fee for biennial renewal of certification is $150.
37(5) The fee for reactivation of an inactive certification is $150, in addition to other required fees.
54(6) The fee for renewal of an inactive certification is $150.
