  • 1(1) All applicants for licensure as a Clinical Laboratory Personnel Director shall apply to the Department on Form #DH-MQA 3008 (05/15) “Application for Clinical Laboratory Personnel Director” 28http://www.flrules.org/Gateway/reference.asp?No=Ref-05607, 29which is incorporated by reference herein. All applicants for licensure as a Clinical Laboratory Personnel Supervisor shall apply to the Department on Form #DH-MQA 3009 (05/15) “Application for Clinical Laboratory Personnel Supervisor” 61http://www.flrules.org/Gateway/reference.asp?No=Ref-05608, 62which is incorporated by reference herein. All applicants for licensure as a Clinical Laboratory Personnel Technologist shall apply to the Department on Form #DH-MQA 3011 (05/15) “Application for Clinical Laboratory Personnel Technologist” 94http://www.flrules.org/Gateway/reference.asp?No=Ref-05610, 95which is incorporated by reference herein. All applicants for licensure as a Clinical Laboratory Personnel Technician shall apply to the Department on Form #DH-MQA 3010 (05/15) “Application for Clinical Laboratory Personnel Technician” 127http://www.flrules.org/Gateway/reference.asp?No=Ref-05609, 128which is incorporated by reference herein. Any licensee requesting to add a specialty to his or her current license shall apply to the Department on Form #DH-MQA 3012 (05/15) “Application for Clinical Laboratory Personnel Adding Specialty” 164http://www.flrules.org/Gateway/reference.asp?No=Ref-05611, 165which is incorporated by reference herein. Copies of all forms can be obtained from the Board office at 1834052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257 192or from the web at http://www.floridasclinicallabs.gov/licensing. The application must be accompanied by the appropriate application fee required by Rules 21164B3-9.001 212and 21364B3-9.002, 214F.A.C.

    215(2) After one year from the date of the original submission of an application, a new application and fee shall be required from any applicant who desires to be considered for licensure.

    247(3) With regard to persons who have been issued a social security number by the Federal Government, disclosure of a social security number is mandatory pursuant to Section 275456.013(1)(a), F.S., 277and social security numbers are used to allow efficient screening of applicants and licensees by a Title IV-D child support agency to assure compliance with child support obligations.
