1The following is a list of acceptable documents which shall be submitted to the Board as appropriate for the type of license sought to show eligibility for the license:
30(1) Official transcript sent directly from the institution.
38(2) A copy of high school diploma.
45(3) Copy of a diploma, training certificate or Department of Defense form DD-214 or any other document which verifies pertinent education and experience.
68(4) A copy of certificate of attendance or documentation of training required pursuant to Chapters 64B3-3 and 64B3-4, F.A.C., and continuing education required pursuant to Chapter 64B3-11, F.A.C., including one (1) hour HIV/AIDS continuing education, and the 2-hour course on medical error prevention.
111(5) Civil notarized copies of documents of foreign education and translation, if appropriate.
124(6) Foreign credentials evaluation which includes a breakdown of all college level courses by credit hours and subject sent directly to the board office by one of the following evaluators:
154(a) A regionally accredited U.S. college or university.
162(b) American Society for Clinical Pathology Board of Certification.
171(c) National Accrediting Agency for Clinical Laboratory Sciences.
179(d) Center for Applied Research, Evaluation & Education, Inc.
188(e) Foundation for International Services, Inc.
194(f) Educational Credential Evaluators, Inc.
199(g) International Consultants of Delaware, Inc.
205(h) Education Evaluators International, Inc.
210(i) International Education Research Foundation, Inc.
216(j) Education International.
219(k) Josef Silney & Associates International Educational Consultants.
227(l) Foreign Academic Credentials Services, Inc.
233(m) World Education Services, Inc.
238(7) Individuals with a baccalaureate degree in accredited medical technology programs from accredited institutions in the Philippines need only submit official transcripts sent directly by the institution to the Board office. No foreign credentials evaluation is necessary.
275(8) Applicants with a degree from a regionally accredited U.S. college or university whose major is not a chemical or biological science may submit an evaluation of the applicant’s academic record from the chairperson of a chemical or biological science department of a regionally accredited U.S. college or university to demonstrate that the applicant’s education is equivalent to a U.S. degree in the chemical or biological sciences.
