64B3-8. Renewal And Reactivation Of Licensure And Inactive Status |
1(1) An approved training program shall renew biennially.
9(2) The training program shall submit a complete form DH5030-MQA entitled Renewal Application for Clinical Laboratory Personnel Training Program, incorporated by reference at Rule 3364B3-3.001, 34F.A.C., and renewal fee to the Department.
41(3) The training program renewal application shall include the following:
51(a) Names of the program director and all instructors, and if applicable the license number of the director and instructors, and submit a resumé, if the director has changed.
80(b) Name, address, license number, personnel roster, and most current licensure or certification survey report of the laboratory sponsoring the training program, if applicable, and all clinical affiliates.
